Service Fee Outline Face to Face/Telehealth Supports Hourly rate $193.99 as per NDIS guidelines This is the time spent with the practitioner.Non-Face to Face Supports To provide quality service, example non-face to face supports that are charged include: - Phone calls and written correspondence to you and others involved in your plan, your needs and your goals e.g. occupational therapist, paediatrician, speech pathologist, psychologist - Report writing, letter writing (if applicable) - Collaboration with other members of care team to determine the best way to help you - Planning, development of program and resources (if required) home enteral nutrition registration, complete nutrient analysis of diet using computer softwareEstimate of Services for a New Client Hourly rate $193.99 as per NDIS guidelines Initial Appointment Duration Estimate: Face to face (in clinic/telehealth or at home): 1-1.5hrs AND Non-Face to Face Supports 1-1.5hrsReview Appointments Duration Estimate: Face to face (in clinic/telehealth or at home): 0.75hrs AND Non-Face to Face Supports are billed specific to clients’ needs AND *Travel duration also charged for home visitsTravel (applicable for home visits only) - Travel is charged for all appointments out of office including to homes, other services or other appropriate location and includes the clinicians time travelled. - Travel will be split between any other participants we see in the tripTerms of Service Agreement This Service Agreement is made for the purpose of providing supports under the Participant’s National Disability Insurance Scheme (NDIS) plan. The Parties agree that this Service Agreement is made in the context of the NDIS, which is a scheme that aims to: • support the independence and social and economic participation of people with disability; and • enable people with a disability to exercise choice and control in the pursuit of their goals and the planning and delivery of their supports.Consent & Acknowledgement: I understand, consent, and agree that: ▪ my personal and medical information may need to be provided to my doctor or other allied health professionals when required to discuss my medical history or requirements, as well as facilitate a referral that I may need. ▪ that my personal and medical information may be provided to authorised suppliers in order to obtain necessary supplies, equipment, appliances or supplements for my care; and ▪ I will be receiving treatment and care from Nourish Paediatrics allied health services and agree to the care plan outlined in this documentResponsibilities of Nourish Paediatrics (The Provider) The Provider agrees to: • Review the provision of supports at least 6-12 monthly with the Participant. • Once agreed, provide supports that meet the Participant’s needs, at the Participant’s preferred times. • Communicate openly and honestly in a timely manner. • Treat the Participant with courtesy and respect. • Consult the Participant on decisions about how supports are provided. • Give the Participant information about managing any complaints or disagreements and details of the provider’s cancellation policy (if relevant). • Listen to the Participant’s feedback and resolve problems quickly. • Give the Participant a minimum of 24 hours’ notice if the Provider must change a scheduled appointment to provide supports. • Give the Participant the required notice if the Provider needs to end the Service Agreement. • Protect the Participant’s privacy and confidential information. • Provide supports in a manner consistent with all relevant laws, including the National Disability Insurance Scheme Act 2013 and rules, and the Australian Consumer Law and keep accurate records on the supports provided to the Participant.Responsibilities of Participant and / or Participant’s representative The Participant and / or Participant’s representative agrees to: • Treat the Provider with courtesy and respect. • Talk to the Provider if the Participant has any concerns about the supports being provided. • Give the Provider a minimum of 24hours notice via phone, if the Participant cannot attend a scheduled appointment (the cancellation policy will apply if less notice is provided). • Give the Provider the required notice if the Participant needs to end the Service Agreement. • Advise the Provider immediately if the Participant’s NDIS plan is suspended, replaced or if the Participant stops being a participant for the purposes of the NDIS. • Inform the Provider about how they wish the supports to be delivered to meet the Participant’s needs.Payments Invoices The Provider will seek payment for the provision of supports after delivery of the Service or Support. All invoices issued by the provider are payable within 7 business days. Select drop down box that applies. 1. [If the funding for any of the supports provided under this Service Agreement is managed by the Participant or their caregiver:] Self-Managed: The Participant has chosen to self-manage the funding for NDIS supports provided under this Service Agreement. After providing those supports, the Provider will send the Participant an invoice for those supports for the Participant to pay. The Participant/next of kin will pay the invoice within 7 days. 2. [If the funding for any of the supports provided under this Service Agreement is managed by a Registered Plan Management Provider:] Plan Manager: The Participant has nominated the Plan Management Provider to manage the funding for NDIS supports provided under this Service Agreement. After providing those supports, the Provider will claim payment for those supports from the Registered Plan Management Provider.Agreement Changes, Cancellations & Appointment DocumentationChanges to this Service Agreement Should any change be required to the supports, services or delivery method agreed to in this Agreement require variation, the Parties agree to discuss and review this Service Agreement. Any variations to this Agreement will be made in writing.The Provider reserves the right to increase the price of any Service or Support provided under this Agreement and will provide The Participant with not less than 28 days written notice of any increase in Service or Support price.Ending this Service Agreement Either party may terminate this Service Agreement with a minimum of 1 months’ notice. This requirement will be waived where it is agreed that either party has failed to fulfill its obligations under this Agreement.Feedback, Complaints and DisputesAll feedback, complaint and disputes are welcomed by Nourish Paediatrics and can be directed to Ashleigh Mitchell Director, via phone on (02) 9453 1888 or email ash@nourishpaediatrics.com.auAll complaints will be managed in accordance with Nourish Paediatrics Complaints Management Procedure. Any complaint involving a reportable incident as defined by the NDIS will be reported to the NDIS Commission. All documentation pertaining to a complaint or reportable incident will be maintained by Nourish Paediatrics for a period of not less than 7 years.If the Participant is not satisfied or does not want to talk to this person, the Participant can contact the National Disability Insurance Agency by calling 1800 800 110, visiting one of their offices in person, or visiting ndis.gov.au for further information. Alternatively contact the NDIS Commission by phoning 1800 035 544 or completing a complaint contact form found online on the NDIS Commission website.Goods and services tax (GST) For the purposes of GST legislation, the Parties confirm that: • a supply of supports under this Service Agreement is a supply of one or more of the reasonable and necessary supports specified in the statement included, under subsection 33(2) of the National Disability Insurance Scheme Act 2013 (NDIS Act), in the Participant’s NDIS plan currently in effect under section 37 of the NDIS Act; • the Participant’s NDIS plan is expected to remain in effect during the period the supports are provided; • the Participant and /or Participant’s representative will immediately notify the Provider if the Participant’s NDIS Plan is replaced by a new plan or the Participant stops being a participant in the NDIS.Minimum Consultation Period The participant agrees that all home visit consultations will be billed for a minimum of 1 hour where an initial assessment is to be undertaken. That is, should a health professional attend your home to undertake a consult of less than 1 hour in length, you will be billed for a 1 hour consultation.Cancellation Policy If the Participant or Participant’s Representative fails to provide more than 24hrs notice (telephone the Nourish Paediatrics clinic number) of a cancellation of a consultation or is not present for the consultation at the scheduled date and time, then 100% of the scheduled fee will apply.Documentation: We document our consultations using a note taking tool called Heidi to accurately and efficiently capture the details of our discussions and the outcomes of our appointments. Heidi ensures that we can focus more on our conversation and less on manual note taking, enhancing the quality of care you receive.
